• Creates, recommends, monitors, and evaluates patient-specific pharmacy care plans for home care patients’ pharmacotherapy. When appropriate, makes appropriate recommendations for changes to a home care patient’s prescribed medication therapy or monitoring parameters. Collects and organizes patient-specific information required for the prevention, detection, and resolution of medication-related problems, as well as the formulation of appropriate medication therapy recommendations. For each patient, appropriate monitoring parameters are identified.
• Based on the evaluation of monitoring data and other pertinent patient-specific information, modifies pharmacy care plans for home care patients. Capable of applying pharmacokinetic principles and formulas. Capable of evaluating patient laboratory values.
• Counsels home care patients and/or their caregivers using effective patient education techniques.
• Ensures that pharmaceutical care is delivered to and from the patient’s home and other care settings. Uses a methodical approach to communicating important patient information to and from the home and other patient-care settings. Prepares medications so that they are appropriately concentrated, incompatible, stable, and properly stored. Follows proper safety and quality assurance procedures. Creates labels in accordance with the organization’s policies and procedures. All necessary and/or appropriate ancillary labels are included with the medication. Before dispensing, inspects the final medication. Assures the presence of a dispense authorization. Complies with state/federal laws and USP standards, including DEA-controlled substance regulations established by the state and DEA. Complies with policies governing perpetual inventory systems for schedule II narcotics, controlled substances, and other diversion-prone medications.
• Interpret medical orders (verbal and written) and accurately transcribe them to computerized patient medication profiles. Maintain complete and accurate patient medication profiles.
• Controlled substances are distributed to patient care areas, and records must be kept in accordance with the law.
• Check drug therapy regimens for contraindications, drug-drug interactions, drug-food interactions, allergies, and drug and dose appropriateness.
• Assist with pharmacokinetics consultation and drug dosing in accordance with organizational protocol.
• Examine/interpret culture and susceptibility data for antibiotic appropriateness and make any necessary changes.
• Accurately read, extract, and interpret information from patient charts.
• Detect and report suspected adverse drug reactions in a timely and accurate manner.
• Maintain the formulary by reducing non-formulary procurement, implementing therapeutic substitution protocols, and encouraging rational drug therapy selection.
• Provide practitioners with clinical consultation and clarification. As needed, suggest appropriate, cost-effective therapeutic alternatives to medical staff.
• Provide professional staff with accurate, adequate, and timely drug information.
• Ensure that all federal, state, and local regulations are followed (i.e. HIPAA).
• Ensure that all company policies and procedures are followed.
• Customer service is provided by greeting and thanking customers in a prompt, courteous, and friendly manner, answering questions, assisting with product location, and ringing up purchases as needed.
• Prepares compounds and dispenses prescriptions for customers accurately and efficiently in accordance with legal requirements (includes product selection, counting and measuring drugs, capping and uncapping vials and bottles and labeling of prescriptions).
• Customers are given instructions for medications prescribed by doctors, and OTC (over-the-counter) medications are recommended.
• Informs the Director of Pharmacy about prescription incidents.
  Verifies that the prescription was written by a licensed prescriber.
  Participate in the administration of immunizations to all patients who are eligible according to our current immunization protocol.
• As prescribed and required, provide Medication Therapy Management (MTM) services.
• Communicates with the Pharmacy Manager about the pharmacy department’s progress and policies.
• In the pharmacy computer system, keeps accurate records of all prescriptions dispensed.
• May photocopy specific prescriptions for additional record keeping; forwards all subpoenas and legal requests to the Pharmacy Manager; and obtains written authorization from the patient before providing records or information.
• Manages pharmacy inventory and reorders medications as needed.
• Ensures that all company pharmacy policies and government regulations regarding medication safety, delivery, and storage are followed.
• Participates in continuing education courses and reviews pharmacy-related literature to stay up to date on new items, changing laws and regulations, and new pharmacy-related programs.
• In the absence of the Pharmacy Manager, directs Pharmacy Technicians, Interns, or Clerks.
• Preparing prescriptions entails entering patient information (demographics, drug allergies, and pharmaceutical history) as well as prescription orders into a computer for processing.
• Ensure compliance with all government pharmacy programs and laws, including but not limited to Medicaid, Medicare Part D, DMEPOS, and HIPPA.
